In preparation for the summer camp season, BSA is rolling out a new, simplified annual health and medical record effective March 1, 2014. Changes have been made based on Volunteer, parent, unit leader, and BSA Professional feedback on how the record could be made easier to use while still collecting the information needed to make sure Scouts are ready for their experience.

Some of the key updates are:    

The primary version will be English. The existing bilingual version will remain available.

Part A (Informed Consent, Release Agreement, and Authorization) will contain no medical information and will be shorter by one page.

Part B (General Information/Health History) will include only the most important information needed.

Part C (Pre-Participation Physical) will be reduced to one page with expanded sections for allergy explanation.

Supplemental Risk Advisory(former Part D) will be shortenedand be location specific.  This is designed to take to physicians so they will better understand what activities their patients will participate in.

A redesign of the landing page for the annual health and medical record website so participants and parents will be able to download only what they need.

ACTION NEEDED:  Please eliminate any and all references to any prior versions of the Annual Health and Medical Record within your unit literature, or websites.  Please delete old versions.

Please save and host only this link:  http://www.scouting.org/scoutsource/HealthandSafety/ahmr.aspx. Don’t print out the record unless absolutely necessary. If you have previously hosted parts of the annual health and medical record on your websites, delete the PDFs, don’t just hide the link.

    After opening the BSA Medical form, I am unable to save as a .pdf version. Is the form locke3d such that it can’t be saved?

      Kendall – Sometimes with fillable .pdfs you have to save them with a different name after filling them out in order to save what you have entered. Give that a try and see if it works.

    I wish you would have put this out sooner.
    I already took my son’s paperwork to the Dr’s and had them complete Part A, B, & C.
    I will fill out some of the new forms but the form that the Dr’s needs to sign will be the old version since I already had to pay once for them to do that. I will not pay again for them to sign new forms.

    I have already sent the forms out to my Pack but will send out the New versions in hope that they did not already do the same.

      Anita-We wish we had received the information sooner that a change was in the works. While we are encouraging anyone who has not already filled out forms to use the new one (since it is easier and clearer), we’re not going to turn people away from things for having the old version.
